Yma will encourage and enable integrated care, designed and delivered around the needs of the citizen.
We will promote the use of technology that enables citizens to access their healthcare records as well as improving communication within the health and social care system.
The use of technology should simplify processes, reduce waste and improve outcomes for the communities we serve.
The Practice
We will support the emergence of an improved Cluster model for Wales where groups of practices have the funding and autonomy to do the work that matters, delivering coherent and equitable offers to communities across Wales, valued by stakeholders across the system.
Improving the resources available to support practice resilience and sustainability.
Widening access and education projects will contribute to the workforce of the future.
Creating the conditions where new roles can be successfully embedded in practice will diversify practice workforce and enable inter-professional learning in primary care.
The Environment
We all need space and time for personal restoration and incubation of new ideas. This feels particularly important in the COVID recovery phase where our health and social care workforce have spent months with the relentless demands of the pandemic.
Yma will help the people working in health and social care to create time and headspace.
We will support people to identify opportunities for change and create the conditions where the individuals willing to drive this change are supported and protected.
